It always bothered me a bit that mental disorders like bipolar are purely phenomenological: they're based on subjective reports rather than biomarkers.
Exactly — there's growing research into biomarkers for depression and other conditions, but I've yet to meet anyone who actually has got comprehensive biological testing. In practice, we're still fully reliant on the phenomenological approach, so when symptoms overlap ... ouch.
